E.R.C.P. IN CASES OF INTRAHEPATIC BILIARY CHANNELS DILATATION WITH A HIDDEN CAUSE AT SONOGRAPHY

Abstract
In the present study, we used ERCP for detection of the cause of biliary obstruction which was hidden at ultrasonography.
The study was carried out on 25 patients, 16 females and 9
males, their age ranged from 21 to 75 years.
All cases had jaundice, some of them associated with abdominal pam vomiting and fever. Ultrasonography was done for all patients where there were intrahepatic biliary channels dilatation in all cases. US could identify the level of biliary obstruction in 92% (23 patients).
ERCP was performed successfully in 88% of the patients. One case was diagnosed without contrast injection.
ERCP identified the level of obstruction in 92 %of our cases (23 patients).
ERCP could detect CBD stones in 85 % of cases(!] out of 13
patients) and sphincterotomy.

ERCP led to a correct diagnosis in 100% of postcholecystectomy patients (7 cases).
ERCP could accurately diagnose 4 of 4 cases (100 %) of cancer head of pancreas. ERCP also diagnosed two cases with ampullary carcinoma, one case Mirizzi's syndrome and one case with cholangiocarcinoma.
The complication rate was 8%. All of them was abdominal pain. No Mortality encountered in our patients.
