Prevalence of Retrograde Ejaculation among EgyptianDiabetes Mellitus Patients

Abstract


Diabetes mellitushas been associated with sexual dysfunction, both in men and women. It is believed that neuropathy, vascular insufficiency and psychological problems may be involved in the pathogenesis of some phenomena. Retrograde ejaculation is defined as the propulsion of seminal fluid from the posterior urethra into the bladder, and it can be either complete or incomplete.
The aim of the work is to determine the prevalence of retrograde ejaculation among diabetic patients attending the diabetes clinic in Ain-Shams University hospital.
This study was conducted on 272 patients attending diabetes clinic in Ain Shams University, during the period between June 2012 and June 2014.
All patients were subjected to proper full history, general examination; blood pressure, temperature, pulse, and respiratory rate, weight in kg and height in cm were measured to calculate BMI and collection and analysis of ejaculates and urine samples.
Forty eight patients (17.6%) had type 1 DM and 224 (82.4%) had type 2, sixty eight patients (25%) received insulin as treatment for DM while 204 (75%) received oral hypoglycemic, sixteen patients (5.9%) had retinopathy, ten patients (3.7%) had renal affection, thirty (11%) had diabetic foot and 216 (79.4%) did not have complications. Regarding erectile dysfunction, it was present in 32 (11.8%) patients. retrograde ejaculation was detected in 32 patients (11.8%), 12 patients of them had ED (37.5%).Eighteen patients were suffering from infertility (6.6%),10 of them had RE (55.5% of infertile cases).
In the current study, urine and ejaculate examination of patients included in the study groups were done,as regards ejaculate volume (3.32±0.96 ml), sperm count in ejaculate (45.47±25.63), sperm count in urine (0.37±1.16), sperm motility in ejaculate (positive in 92.6% of patients) and sperm motility in urine (positive in 10.3% of patients) indicating prevalence of retrograde ejaculation in the study groups).
No significant difference between groups was foundas regardsalcoholism (P= 0.462), while age showed significant difference between both groups (P=0.032).
Results of the current study show that significanthigher body weights and BMI were detected in group 1 compared to group 2 (P=0.001, 0.002 respectively), also significant lower height in group 1 compared to group 2 was detected (P=0.000).
Results of the current study show significantly higher systolic blood pressure, diastolic blood pressure a, fasting blood sugar and HbA1cin group 1 compared to group 2 (P=0.000, 0.001, 0.000, 0.002 respectively).
Results of the current study show significantly higher smoking rate in group1 compared to group 2 (P=0.001).
Results of the current study show significantly higher percentage of ED and infertility in group 1 compared to group 2 (P= 0.002, P= 0.002respectively).
