THE PREDICTIVE VALUE OF PREGNANCY SPECIFIC BETA-1 GLYCOPROTEIN AND PREGNANCY ASSOCIATED PLASMA PROTEIN-A IN THE FIRST TRIMESTER THREATENED ABORTION

Abstract
This case control study was conducted to assess the value of
pregnancy specific -1 glycoprotein (SP1) and pregnancy associated plasma protein-A (PAPP-A) either separately or in combinations in
predicting the pregnancy outcome of the first trimester threatened abortion. It was found that, by using cut off values of(53.2 ng/ml) for SPI and (52.3 ng/ml) for PAPP-A, the best overall perfonnance of SPI or PAPP-A was obtained in the followinorder:
1- The combination of SP I 2n samples + PAPP-A 1'1 or 2nd
samples (diagnostic sensitivity 92.85%, diagnostic specificity 100%, prediction of abortion 100%, prediction of successful outcome 94.11% and diagnostic efficacy 96.66%).
2- The combination of SP1 1'1 samples +PAPP-A 1'1 samples
(diagnostic sensitivity 85.71%, diagnostic specificity 100%, prediction of
abortion I 00%, prediction of successful outcome 88.88% and diagnostic efficacy 93.33%).
3- SP I 2"d samples or the combination of SP I I st + 2"ct samples
(diagnostic sensitivity 92.85%, diagnostic specificity 93.75%, prediction of abortion 92.85%, prediction of successful outcome 93.75% and diagnostic efficacy 93.33%).
4- PAPP-A I st samples, PAPP-A 2nd samples or the combination
of PAPP-A 1'1 + 2"d samples (diagnostic sensitivity 85.71%, diagnostic specificity 93.75%, prediction of abortion 92.30%, prediction of
successful outcome 88.23% and diagnostic efficacy 90%).
5- Lastly, SPl 1'1 samples (diagnostic sensitivity 57.14%, diagnostic specificity 93.75%, prediction of abortion 88.88%, prediction of successful outcome 71.42% and diagnostic efficacy 76.66%).
In conclusion, SP I and PAPP-A were found to be highly
encouraging in predicting the pregnancy outcome of the I st trimester
threatened abortion.
