EFFECT OF ANGIOTENSIN CONVERTING ENZYME INHIBITORS VERSUS LONG ACTING CALCIUM ANTAGONISTS IN TREATMENT OF CHRONIC AORTIC REGURGITATION

Abstract

+ The basic cause of chronic AR in our society is RHD.

• Chronic AR is more prevalent in males. The area of gender difference in patients with chronic AR needs to be more stressed, also, is waiting for explanation. A larger
"
population based survey may help this point, especially in
. . \ .
our society, and provide the important lacking figures of

'the prevalence and true magnitude of the problem of
,•,
rheumatic valvular heart disease.

• Until now, nifedipine has been recognized as the most effective among vasodilators in AR patients. On the basis of their. vasodilator mode of action, ACE inhibitors-which have been shown to be highly effective in terms of their management of hypertension and heart failure- could be expected to produce equally positive effects in the
• treatment of AR patients as were seen with nifedipine.

• In studies of long-term vasodilator therapy in patients with chronic AR, a reduction of left ventricular volumes and regurgitant fraction, with or without an increase in ejection fraction, has been observed during treatment with hydralazine, nifedipine and ACE inhibitors.
