THROMBOLYTIC THERAPY IN MANAGEMENT OF PERIPHERAL VASCULAR THROMBOSIS
AMR AHMED MOHAMMED EL-SAID;
Abstract
Peripheral vascular thrombosis (arterial and venous) is a common medical problem that leads many complicatins and associated with high morbidity and mortality.
Although, there are traditional ways to treat this problem, recent ways of treatment are available now a days like endovascular and thrombolytic therapy.
A strategy of early clot removal is effective at preserving limb function and alleviating clinical symptoms. Thrombolysis has proved efficacy as adjunctive treatment in acute deep venous thrombosis and acute thrombotic limb ischemia .
Newer thrombolytic agents provide more efficacy in the form of rapid clot lysis, higher safety in the form of less bleeding complications
Catheter-directed therapies have been shown to be safer and more effective than systemic infusions. Catheter-directed thrombolysis allows concentrated delivery of lytic agent, maximizing local effect while minimizing systemic bleeding complications.Mechanical devices, often used as adjuncts to lysis, offer the potential to extract clot even more efficiently and with added safety.
Using Angiography is mandatory before and after the procedure. angioplasty could be done in case of visualized stenosis or even stent placement. Repeated venography or arteriography confirms patency of the vascular tree.
Finally , my clear message from this work is : if you face a case of peripheral vascular thrombosis ( arterial or venous) , keep in your mind thrombolysis as an available line of treatment.
