A Comparative Study Between Single-Incision Laparoscopic Surgery (SILS) and Conventional Laparoscopy in Appendectomy

Abstract


The present study included 40 patients admitted to surgery department in Ain shams university hospitals and El-Agouza hospital, complaining of acute appendicitis.
The aim of the study was to make a comparative study between conventional laparoscopic Appendectomy and Single port Laparoscopic Appendectomy to compare the two techniques as regard the operative complications and the post-operative outcomes.
All patients were subjected to detailed history taking, complete physical examination including routine and specific laboratory tests, ultrasonography examination of the abdomen.
The patients were divided into two groups each was 20 patients, appendectomy done by the two methods of operations; Group (1): Convensional laparoscopic appendectomy (CLA), Group (2): Single port laparoscopic appendectomy (SILS).
The patients were assessed for:
• The operative time in minutes.
• Post-operative hospital stay till discharge in hours
• Post-operative pain by a Visual Analogue Scale (VAS).
• Intra-operative complications.
• Post-operative complications.
• Patient’s satisfaction and cosmesis.

The results of the study were:

The operations were completed successfully by laparoscopy in 39 patients without conversion and only one patient had conversion.
One case with SILS was converted to open appendectomy due to mass formation.
There was no conversion to open appendectomy in CLA group.
In the first 5 cases the operative time in SILA was more than 60 minutes because it took time by the operative team to get adapted to the new position of the instruments and difficulty in manipulating instruments, but later on with performing more cases the time decreased to a range from 40 to 60 minutes with a mean of 55.8 minutes in the SILA group, and was 51.7 minutes in the CLA group.
The overall postoperative complication rate was 15 % (n = 3) for the CLA group and 25% (n = 5) for the SILS group.
There was a certain cosmetic advantage of performing the single port surgery compared to the standard procedure.
From the above mentioned results, SILS appendectomy is considered to be a comparable route to the conventional laparoscopy approach from all points of view with the super added merit of better cosmesis and patient’s satisfaction.
