Digital camera assessment for detection of cervical intraepithelial neoplasia

Abstract


Cervical cancer is the third most common cancer among women in the world (Howlader et al., 2012).
The human papilloma virus (HPV) is the main causal factor for the development of invasive cervical cancer (CC), and HPV is found in nearly 100% of these tumors. CC results from the progression of pre-invasive cervical intraepithelial neoplasia (CIN) (Schiffman et al., 2011).
There is a need for new diagnostic tests that would either supplement or replace cyto-diagnostics as a screening tool (Rokita et al., 2012).
The aim of this work is to establish value of digital camera assessment of the reproductive tract (DART) as a method for screening of cervical cancer.
This is a cross sectional study that was conducted in the ECDU at ASUMH during the period from December 2009 to December 2013.
Equipment:
1. A 12 mega pixel camera was used (Sony cyber-shot), with 4X optical zooming capacity and full high definition (HD) property. The cost of the camera is 1700 Egyptian pounds.
2. A Welch Allyn Kleenspec plastic disposable vaginal speculum (medium size) was used with an attached halogen illumination system. The cost of the illumination system is 750 Egyptian pounds.
3. A Carl Zeiss West Germany colposcope was used.
2215 patients were enrolled.
All patients were selected at random from those attending the ECDU at ASUMH.
After explanation of the procedure to the patient and reassurance that the procedure is painless, the patients were positioned in lithotomy position.
After identification of the metaplastic squamous epithelium, squamous epithelium, the squamo-columnar junction and the TZ, a photo using the digital camera was taken, then a pap smear.
Application of 5% acetic acid to the cervix was done, the cervix was observed for 30 seconds for aceto-whiteness.
This was followed by capturing another photo. Then lugol's iodine application and another photo.
All photos were taken at 1.7 X optical zoom with the camera adjusted to focus at near objects and it was manually held just at the vulva. This was followed by history taking.
