NEUROSURGICAL MANAGEMENT OF I FOURTH VENTICULAR TUMORS INI CHILDHOOD

Abstract


The fourth ventricle is a very common site for intracranial neoplasms in children. The commonest ages affected are usually between 7 and 13 years.


Though Medulloblastomas and Ependymomas have been recognized as the most commqn tumours in this area other neoplasms can also occur.


They most commonly present with manifestation of increase intracranial tension due to secondary obstructive . hydrocephalus which usually brings the child with a foUith ventricular tumour to the neurosurgeon. The typical history is headache, vomiting and lethargy or drowsiness.


Radioimaging has progressed markedly in the past decade and no longer is CT scanning the most accurate facility of diagnosis, but MRI has superceded and can be used to it asses diagnosis radiologically. But CT still remains dependable and can be useful for evaluation and follow up of hydrocephalus.


Marked controversy still exists regarding the initial management of a child with a fourth ventricular neoplasm.


Some advocate routinely shunting the hydrocephalus as a first step. The problems and potential risk fi•om shunt placement together with the lack of any benefit are against shunting.
