A comparative study of two different techniques of intraocular lens implantation in pediatric cataract

Abstract


This is a prospective study carried out on 30 eyes of 24 patients divided randomly into 2 groups: 15 eyes in group A underwent optic capture of the IOL and 15 patients in group B underwent in the bag implantation of the same IOL.
There was highly significant statistical difference (p<0.001) between preoperative and postoperative of BCVA in both groups. Comparing between both groups along the whole postoperative period showed no statistically significant difference in BCVA (p= 0.868).
Preoperative results ofintraocular pressure (IOP) during follow up period showed no significant difference in the p-value in both groups across time (p = 0.367) and no significant difference in the P value in between different studied groups (p = 0.555).
Spherical equivalent power 1 week postoperative and 6 months were not found to be not statistically significant when reviewed over time with (p=0.226) also no intra group difference. (p=0.405)
Regarding clear central zone was measured as an end point result at the 6 month follow up, results were statistically significant. (p =0.001)
Clarity of the ocular media was also evaluated according to grading of red reflex clarity in both groups over 6 months follow up period. The red reflex in group A and B as an end point comparison. Results showed no statistical significance in the studied groups. (p=0.181)
Centration of the IOL was observed at each postoperative visit and documented. Results at 1st week were not statistically significant (p= 0.483) second and third visits were statistically significant.
With (p=0.042) at 1 month follow up and a high statistical significance at 6 month follow up. (p=0.017)
Complications among studied groups:
• Postoperative occlusion of visual pathway:
YAG laser sessions needed to manage complete central occlusion was only needed in 3 cases of group B while no cases in group A were in need of intervention of that kind.

• Intraoperative IOL displacement:
A single case with intraoperative IOL displacement on attempted capture in capsulorhexis. The IOL was repositioned in the bag intraoperative and no more complications were observed in this patient during postoperative period.
