Abstract
Yoder and associates demonstrated that non-anatomic superior and lateral
.placement of the acetabular component resulted in significantly high rates of femoral loosening. (Yoder, S.A.; Braud, R.A. & Pedersen, D.R.; 1988). If part of the acetabular cup is not covered by load-transmitting bone, the stresses in the cement, the bone and especially the cement-bone interface are increased. Central wall defects of the acetabulum can be reconstructed using cancellous strips. Major central or superior intra-acetabular defects where structural support is important can be reconstituted with the patient's own femoral head or, if that is not available, allogeneic bone. Femoral heads provide excellent material for reconstruction of these rim or major intra-acetabular defects. They usually are used in bulk and are fixed to the pelvis with appropriate instrumentation.
