Troponin-T In Unstable Angina: Diagnostic and Prognostic Implication

Abstract


o examine the diagnostic and the prognostic ro l e of Troponin­ T in patients admitted with unstable angina, we conduced a double blinded, prospective study on 32 patients admitted wi th
chest pain suggestive of acu te unstable angina (class III) and followed up for a six months. Study popu l ation involved 23 males, 9 females with a mean age of 51.2±5.


For fol low up, readmission with non-fatal myocardial infarction, recurrent admission wit h unstable angina, progression of unstab le angina to myocardial infarction or need for intervention were considered end poin ts


Foll owing clinical examination and routine laboratory invest igat i ons, all patients had serum determination of CK cti vity, the myocardial band (i soenzyme MB) acti vity and quantitative cardiac Tropon in-T, on admission, 8 hrs, 24 hrs, 48 hrs after admission to the hospital. Twel ve lead ECGs were obta i ned for a ll patients on admission, twice daily and whenever needed. Coronary arteriography was perfom1ed for all pts predischarge except four pts not performed.


Out of the 32 pts, 22 (68%) were Troponin-T positive. Recent myocardial infarction occurs in six (24%) of twenty-two Troponin-T
