Abstract
Astigmatism IS a frequent problem postoperati vel y in otherwise successful ' penetrating keratoplasty. Each of the multi ple steps in pe rforming penetrat ing keratopl asty f ro m traph ination of the t i ssu e to the fi nal sutu ring tech nique may affect final asti gmati c results.
F i rst, we have discussed t he diffe rence techniques used 111 meas urement & cvalu:Hion of astigmat i sm. Re tin oscopy and or s u bjective reli•acti on yeilds the total refraction of the eye, w h ich includes le nti cula r astigm atism . Kerato metry provi des i nforma tion regarding the central 3 millimetes of corneaJ..curvature but does n to descri be i rregula r astigma ti sm well. Corneal topography provides
t he most useful and complete i nformation.
Reduction of postkeratoplasty astigmat i sm is founded i n two suturing techn i ques. First techn ique is a single run nin g sutu re with sutu re ad j ustment. Second technique is combi ned running a nd in terrupted suture wit h selective suture removal.
By co.mparing the two tech nique, adjustmen t of the si ngle running suture can reduce ea rly postoperative asti gmati sm to a greater extent and w i th less suture man i pu l ation tha n sel ective rem ova l of in terrupted sutures. Thi s attribu ted to more effecti ve redi s tribution of t he corneal tension a l ong the entire circum ference
or t he wound.
