FACIAL NERVE NEUROGRAPHY IN VARIOUS FACIAL NERVE LESIONS

Abstract


The anatomy of the facia\ nerve, \ts \ong course in a re\ative\y narrow bony canal and the single large fascicle that makes its temporal po11ion makes it particularly vulnerable to damnge. The most common cause for its affection is Bell's palsy (V/ilw...lii el 11/., 1984).


ENoG is used to determine the physiologic status of the facial nerve. The term ENoG refers to the electric stimulation of the nerve (liecl< tllltf liencclw, 1993). It is the recording of the compound muscular action potential fro111 the facinl muscle allcr stimulation of the nerve or one of its branches by surface electrodes (Cramer and Kartuslt, 199/).This CMAP results from the synchronous firing of multiple motor units (Ke/lelrer el nl., 19911). 1t is currently the most useful electro-physiologic test of facial nerve fUnction

It is an easily accomplished maneuver and requires no anesthesia, non invasive, quick, clTicient and highly reliable. It is also objective and quantitative measure of the nerve function (Eng.'tfrom et ul., 2000). And it can be interpreted within the context oft he clinical presentation and the patient's relevant hist01y (May ct a/., 1983).


The stimulating electrode was at the stylomastoid foramen, while the recording electrodes were at both alae nasi with the active electrode on the ipsilateral side and the reference at the other side. This was convenient with the sites chosen.by ,\'mith et nl.,
1988. Also it was in agreement with that of Mny el at., 1985 and reported to be most convenient and reliable recording sites by Kartush et nl., 19RJ.


Filly seven individuals of both genders (39) males and ( 18) females were tested with ENoG. Fifieen of them were as a control group and (42) as a study grm1p, twenty one of them as group (A) and the other twenty one as group (B). Their ages ranged from
4 to 56 years. With a mean of34.6 ± 12 years for the control group and 30.9 ± 14.5 years

for both study groups.
