FAILED BACK SURGERY SYNDROME

Abstract


Failed back surgery syndrome is a term used to collect certain group of patients with persistent or recurrent of their symptoms (back pain andor sciatica) in spite of previous lumbar spine surgery.
Chronic low back pain is one of the most prevalent conditions affecting the working population of today’s society. Impairment of the back and spine are the most frequent cause of limitation of activity in persons less than 45 years old.
Many studies done on different cases of failed back surgery syndrome were surgically corrected by either decompression and/or fixation.
From the result of these studies the following be concluded:
-It is common in middle age males due to higher physical activities.
-The most common causes of failed back surgery syndrome are :Improper selection of the patient whether which patient who get benefit from the operation and which level or levels should be operated and type of the operation (decompression and/or fixation).
-Bad surgical technique and bad handling of the tissues and bad management of the complications.
-The most common symptoms were back pain and sciatica denoting muscle spasm and nerve root compression by disc fragment.
-The most common signs were sensory testing and straight leg raising test denoting nerve root compression.
-Magnetic resonance imaging with contrast was the investigation of choice and remaining so in differentiation of recurrent fragment or fibrosis.
