Short-Term Regression of Ventricular Mass after Aortic Valve Replacement in Aortic Stenosis with Left Ventricular Concentric Hypertrophy –Egyptian experience

Abstract


Aortic valve replacement has produced a dramatic change in the nature history of AS. Measurement of the degree of LVH in patients with AS, particularly the assessment of the regression of such hypertrophy after valve replacement is of major importance. Regression of left ventricular mass after aortic valve replacement may reduce the long-term complications of sudden death and congestive heart failure associated with left ventricular hypertrophy. In the previous studies, it was found that LVH regression is a time-consuming process and occurs over a period of several months. We studied 30 patients with isolated AS who underwent AVR by mechanical valves. We measured LV dimensions, LVM, LVMI as well as Mean, Max PG at the aortic valve. We found that there was a significant reduction in LV dimensions immediately post operative followed by less reduction at 3 month follow up, more evident in LVEDD, LVESD. The Aortic valve Max PG was significant reduced immediately post operative (70.30±16.51%) followed by further but less reduction at 3 months follow up (5.95±1.50%). Similarly The Aortic valve Mean PG was significant reduced immediately post operative (60.13±9.50%) followed by further but less reduction at 3 months follow up (1.95±0.83%). the LVM& LVMI showed significant reduction immediately post operative followed by further less reduction at 3 months follow up (20.61±14.26% to 14.08±10.50%) and (19.86±14.62% to 12.95±10.19%) respectively. And this was comparable to other studies which showed similar results. However, dimensionless index that also showed increase immediately post operative& at 3 months follow up and may be the most helpful index for prosthetic valve stenosis.
