and review all the researches&the literatures concerning with this subject.Aiso the evolutionary designed IOLs and the best postoperative results for the patient, the surgeon and the community.
Phacotmesis:
The objective in the development and evolution of Tmesis was to develop an inherent mechanism in the instrument itself.The thoughts were to somehow find a way of making the instrument differentiate between the structurally friable nuclear particles and smooth capsular membrane, attacking the former and protecting the latter .The initial idea was to use a ring that rotates at high speeds around its diameter it would connect with an aspirating cannula in line with its diameter.Its advancing edges as it rotates would be sharp and its receding edges blunt.The way it worked was as it rapidly rotates it would form a hollow sphere that opened into the aspirating cannula.when brought into contact with nucleus, it would shave slivers from the nucleus in a tangential direction (hence the term "Tmesis,"Greek for cutting)and these slivers within the
hollow sphere would be immediately aspirated cavitation (
leading to further collapse of the nuclear matter.
