Study of the Tracheal Ecology in Intensive Care Patients Before and After Mechanical Ventilation

Abstract


For this study 30 patients connected. to mechanical ventilation were selected for serial X-ray chest, leucocytic count and BALF by fibroptic bronchoscopy to diagnose new invading tracheobronchial organisms.
We found a direct relation between bypassing defence mechanisms in the orolarynx and duration of MV with the invasion of the tracheobronchial tree with new microorganisms and they are: staph. Aureus 33.33%, E.coli
33.33%, P.pyocyanes 27.77% and strept. Pneumonia 5.55%.
Also we found that the more the duration ofM.V. increased the more TLC will increase due to the invasion with the new microorganisms.
We found that the X-ray chest new infiltations is diagnostic for the new tracheobronchial invading microorganisms and a guide to take the BALF samples from the affected bronchus and its lung segment.
Recommendations
We recommend to repeat this study on a large scale in different centers to compiar between the different microorganisms in the tracheobronchial tree at different environment.Aiso we recommend to repeat this study on the mechanically ventilated patients using antibiotics and non using antibiotics from the start of MV to evaluate the efficacy of the use of this antibiotics.And to repeat this study with the use of different modalites of ven'tilator bacterial filters and to study the tracheobronchial flora m mechanically ventilated patients.
