ELECTIVE FIBEROPTIC ENDOSCOPIC INJECTION SCLEROTHERAPY AS COMPARED WITH BAND LIGATION FOR LARGE ESOPHAGEAL VARICES

Abstract
Bleeding from esopha,Jeal varices is a major complication of portal hypertension and carries a.n extremely high mortality. The presence of different modalities for the r.1anagement of esophageal varices means that no one is fully accepted. Although endoscopic injection sclerotherapy (EIS) is currently the most widely used treatment for the management of patients with esophageal varices, it is associated with various local and systemic complications that may limit its effectiveness.
In attempt to reduce the incidence of these complications, another endoscopic technique employing different principle has been explored. Based on the technique o'' band ligation of haemorrhoids, band ligation of esophageal varices has been developed and adapted for use with the flexible endoscope.
The aim of the pre:;ent work, therefore, was to evaluate efficacy, safety and incidence of complications after endoscopic variceal ligation (EVL) as compared with EIS.
To achieve this goal, forty patients with cirrhosis and/or schistosoma! hepatic fibrosis who had a definite history of variceal bleeding from esophageal varices dia!Jnosed during endoscopy were included in this study and were randomly allocated into two matched groups:
Group 1: received r13peated EIS with 5% ethanolamine oleate intravaricea.lly and sometimes paravariceally till variceal eradication.
Group II: received repeated EVL till variceal eradication.
