Abstract


Bleeding esophageal varices as a complication of portal hypertension is the most common cause of upper gastro - intestinal haemorrhage in Egypt. Schistosomal hepatic fibrosis with portal hypertension is the leading cause of death in patients Egypt. The economic problem of this disease is aggravated by the fact it affects individuals at peak of their productive life.

This work aimed to study haemodynamics of portal vein, splenic vein and hepatic artery before and after splenectomy & devascularization and esophageal transection operations in cases of chronic liver disease with portal hypertension and bleeding esophageal varices.

This work included fifty patients suffering from liver cirrhosis with splenomegally and portal hypertension and bleeding esophageal varices. All patients were admitted to department of general surgery and submitted to thorough history taking, clinical examination, laboratory investigation and upper gastro oesophageal endoscopy to detect the degree of esophageal varices. Forty patients were subjected to splenectomy & devascularization (group I), and ten patients were subjected to esophageal transection combined with splenectomy & devascularization.
