COMPARATIVE STUDY BETWEEN ONLAY, INLAY AND SUBLAY MESH USED IN REPAIR OF INCISIONAL HERNIA
Ahmed Helmy Aly Youseff;
Abstract
Incisional hernia is common surgical condition well known to be associated with occurrence of serious complications. In addition, the occurrence of. these hernias which are very common in obese patients, may be quite disfiguring.
Surgery remams the only modality of treatment. Since the dawn of surgery this condition is known and managed. Following William j. Mayo who promoted in 1898 his well known "Mayo 's" repair, different techniques have been described to deal with these hernias. Surgical mesh was later also used to repair these hernias.
However, still there IS no final good method for management, this is attested by the high incidence of recurrence inspite of the different techniques used.
The anterolateral abdominal wall musculofasciallayer is a functional one unit. It is mentioned that ventral hernias in many cases represent a local expression "local failure" of a generally weak musculofasciallayer.
More than one study was done to understand the hernial mechanism and how to repair it. Askar in 1984 pointed out the following:
