LAPAROSCOPIC CHOLECYSTECTOMY IN ACUTE CHOLECYSTITIS

Abstract


The role of laparoscopic cholecystectomy in the management of acute cholecystitis remains controversial. After gaining more experience in this new technique, many skilled surgeon started to treat acute inflammation of the gallbladder laparoscopically with promising results.
In this study 30 patients were included, in whom acute cholecystitis was diagnosed preoperatively by history taking, thorough clinical examination, laboratory investigations, ultrasonography and radiological examination.
Intraoperatively, the diagnosis was confirmed by the presence of inflammation with peri-cholecystic oedema, gallbladder distension, mucus or pus on aspiration of gallbladder.
Twelve patients had scars of previous lower abdominal operations, and one upper abdominal scar. Associated medical diseases were present in 12 patients which were controlled pre­ operatively.
All patients were subjected to laparoscopic cholecystectomy during the first 24-72 hours of admission under umbrella of antibiotic and I.V. fluids.
The standard four cannula technique was used in all patients
