Treatment of Trochanteric Fractures in adults

Abstract


In the above essay; we reviewed in brief all about the treatment of the fractures in the Tochanteric area; starting from the gross, vascular, skeletal and surgical anatomy passing through the regional biomechanics and the forces acting on this area and the effect of the muscles' contraction and so on. We then shifted to the classification of these fractures according to Boyd: classification into Types I, II, III & IV and Evan's classification into stable and unstable fractures. Furtherwards came the role of clinical • assessment of the • patient (mainly shortening and external rotation deformity) and radiological assessment by plain• x-ray, with a brief-hint on Singh's index of osteoporosis and its grades (from l ---> 6). Reaching our subject of concern and great interest, we started reviewing the different modes of treatment starting by the conservative one until reaching the different fixing devices used with examples on each. We mentioned the special cases in which the hemiarthroplasty operation was employed in the treatment of Trochanteric fractures. Complications were also reviewed whether intra or post operative immediate•or delayed or even those related to the conseryative treatment. At the end; a brief review of the results
of treatment in relation to the different techniques was done followed by the discussion and the authors' opinions and the references used.
