AMBULATORY BLOOD PRESSURE MONITORlNG IN PATIENTS WITH CHRONIC RENAL FAILURE

Abstract


We carried out this study on 90 patients who are subdivided into three groups,group 1(30 patients)with CRF on hemodialysis
,group II(30 patients) with CRF not on dialysis and group III (30 patients) with essentiai hypertension. Casual blood pressure measurment and ABPM was done for the three groups. ABPM was carried out for one day for group II and group III and for three days for group I(the day before dialysis-the day of dialysis- the day after dialysis ).

In this study we found that hypertension is more prevalent in CRF patients specially those on hemodialysis. ABPM was superior to casual blood pressure measurement in the diagnosis of hypertension in patients with CRF also in detection of non dippers . which ts an early prediction cardiovascular complications . ,

Also in this study we found that glomerulonephritis was the commonest cause of renal failure and associated hypertension followed by diabetic nephropathy and hypertensive nephropathy.


Patients on chronic hemodialysis for long durations were found to have higher BP than those on hemodialysis for short durations. Also we found that there was a decline in the mean arterial pressure in the day of dialysis • and the day after compared to the day before dialysis specially in patients with hypertension and those with interdialytic weight gain more than
2Kg.
