Abstract


oninvasive positive pressure ventilation (NIPPV) has
been increasingly used in the management of
respiratory failure in intensive care unit (ICU). In the present study, Our aim is to compare the efficacy and resource consumption of NIPPMVdelivered through face mask against invasive mechanical ventilation (IMV) delivered by endotracheal tube in the management of patients with acute respiratory failure (ARF).

This is a prospective randomized controlled study including 78 adults with acute respiratory failure who were admitted to the intensive care unit. The enrolled patients were randomly allocated to receive either noninvasive ventilation or conventional mechanical ventilation (CMV).

The study eligibility criteria were patients with acute respiratory distress whose condition deteriorated in spite of comprehensive medical management, including severe dyspnea and a ratio of the partial pressure of arterial oxygen to the fraction of inspired oxygen (PaO2:FiO2) < 200 while the patient was on 40% oxygen therapy using a Venturi mask and paco2 higher than 45 mmHg, We excluded patients who had any of the following criteria: immunosuppressive therapy, respiratory failure secondary to neurological disease; respiratory arrest, urgent intubation for cardiopulmonary
