Biochemical Study on Nesfatin-1 in Type 2 Egyptian Diabetic Patients with and without Complications

Abstract
Diabetes mellitus describes multiple etiologies of metabolic disorder which is characterized by chronic hyperglycemia accompanied with disturbances in the metabolism of carbohydrate, fat and protein which results from defects in insulin action, secretion or both.
In this study, urine and serum samples were collected from males with T2DM (n=25), T2DM with nephropathy (n=22), T2DM with cardiovascular disease (n=22) and healthy volunteers (n=19). The analysis included the parameters of matching for BMI, Glycemic indices, lipid profile, and kidney function tests. Insulin, NAG, vWF and nesfatin-1 estimation were also determined quantitatively using ELISA technique. In addition, traditional anti-atherogenic indices were calculated.
Data obtained were summarized as follows:
• Glycemic indices: Both the FBG and HbA1C showed a significant increase than the control group with the highest increase seen in DN, followed by DC and finally T2DM,with percent change of 82.2, 154.9 and 125.9 %, and HbA1C was 46.8, 87.6 and 57.04 % for T2DM, DN and DC groups, respectively.
• Insulin related indices: The insulin and HOMA-IR showed significant increase, while HOMA-β showed a significant decrease than the control group, with the highest decrease seen in DC followed by DN and T2DM. The % change of insulin (20.5, 127.8 and 123.52 %), for HOMA-IR (118.72, 452 and 410.9 %) while for HOMA-β was (-59.3, -56.3 and -49.6 %) for T2DM, DN and DC, respectively. With these results, HOMA-IR and HOMA-β could be taken as valid tests for diabetes.
• The kidney function tests: The current result showed that s.urea, µ alb.uria, ACR, s. urea and NAG showed significant increase in the pattern of DN>DC> T2DM in all of them while s. creat. showed increase in the following manner DN, T2DM and DC. % change of s. creat. was 10.52, 30.5 and 4.2%, µ alb.uria was 4.58, 5685 and 76.6 %, ACR was 18.2, 9222 and 123.24%, s.urea was 2.55, 16.9 and 12.19 while NAG % change was 0.8, 6.45 and 4.02% for T2DM, DN and DC, respectively.
