Abstract
ransannular patch repair has dramatically reduced death in TOF. However, the paradigm for surgical repair of TOF has shifted. A prior strategy focused exclusively on relief of PS has changed into one that additionally aims to avoid the now known long-term deleterious consequences of pulmonary regurgitation. Consequently, many surgeons now favor valve-sparing approaches and have extended the boundaries of repair toward PV z scores that have previously been considered too small to spare. An optimal repair would restore normal RVOT anatomy and physiology, a PV which functions normally without PS.
