HEMATOLOGICAL MANAGEMENT OF PATIENTS WITH CONGENITAL CYANOTIC HEART DISEASE

Abstract


Abstract
The study included 50 patients with CCHD from Abou• Elreesh Children's Hospital; 27 males and 23 females. All 50 children were submitted to: full history, clinical examination, X-ray chest and heart, ECG, echocardiography, CBC, serum iron, total iron binding capacity(TIBC), serum ferritin, transferrin saturation percentage(TS%) and blood viscosity test. All patients were diagnosed as anemic as they all had a hematocrit(Hct)less than 60%, since the diagnosis depends on the Hct level being less than 60% or hemoglobin(Hb) level less than 15gm/dl. They were all given iron for one month to return for follow-up. 25 patients (half of them) returned for follow-up after that month and were submitted to CBC(full blood picture).
The results of the study showed the following: The majority of cases had normal levels of Hb(52% of cases), Het(56%), MCV(48%), MCH(54%), MCHC(56%), RBC count(50%), WBC count(90%}, platelet count(58%) and serum ferritin(54%); The majority of cases had higher than normal levels of serum iron(54%), TS%(60%) and blood viscosity( 100%); While the majority of cases had lower than normal levels of TIBC(54% of cases). Also there was no improvement in any of the blood indices of the CBC after iron therapy except for the WBC count which decreased but within normal range). In addition, there was a significant positive correlation between Hb and Hct and between blood viscosity, Hb and Hct; and a significant negative correlation between TIBC and TS%.
