Selectivity Index as a diagnostic and prognostic marker in Nephrotic Sgndrome

Abstract

SUMMARY
Nephrotic syndrome (NS) is one of the common renal diseases in children. It is characterized by the presence of proteinuria, hypoproteinemia, edema, and hyperlipedaemia.
NS is a disorder of glomerular perm selectivity that may be primary or secondary to a systemic disease.
Most of the NS patients go into complete remission after steroid therapy "steroid responsive ", represented in this study by group I. However, a group of patients develop two consecutive relapses during steroid treatment or within 14 days of its cessation "steroid dependence", represented by group II. Others fail to achieve response to steroid in spite of 4-8 weeks of treatment "steroid resistance", represented by group III.

Although renal biopsy is the definitive investigation in some renal disorders and is helpful in distinguishing steroid responsive NS from steroid resistant; it is attended by some risk to the patient and is contraindicated in certain conditions. Accordingly, some non-invasive tests have been used to predict the response to steroids and the underlying renal histological diagnosis in NS.
