PROFILE OF OPEN AND CLOSED HEART SURGERY IN ASSIUT UNIVERSITY HOSPITAL

Abstract
this work displayed a11 our cardiac surgical work in the cardiothoracic surgery department of Assiut university hospitals in the last ten years which included 1131 cases subjected to operative interference either in the form of open heart operations (694 cases) or closed heart operations (437 cases).
Our patients were 488 males (43%) and 643 females (::7%) and their •
age ranged from 4 months to 44 years old with a mean age of 29.4 years.
The pathological lesions in cases older than IO years old were mainly valvular lesions as mitral valve stenosis (39%), mitral insufficiency ( 18% ), aortic regurgitation (17% ), aortic stenos is ( I 5%) and combined mitral and aortic lesions (11 %) in addition to 116 cases of pericardia I lesions.

On the other hand all cases younger than IO years old were operated for congenital heart lesions either simple lesions as patent ductus arteriosus (204 cases), atrial septal defects (3 7 cases), ventricular septa! defects (9 cases), and subaortic membrane (JO cases) or complex lesions as tetralogy of Fallot (8 cases), atrioventricular canal defects (3 cases). and pulmonary stenosis associated with ASD (3 cases) or with YSD (5 cases).
The 288 cases of congenital heart lesions (25% of all cases) were operated either by open-heart surgery (75 cases, 26%) as atrial septa! defect, ventricular septa! defects, tetralogy of Fallot and atrioventricular canal defects with the youngest age was 3 years old patient or by closed heart techniques (213 cases, 7 4%) as patent ductus arteriosus and coarctation of aorta.
Of the remaining 843 cases (75% of all cases), there is 619 cases (73%> operated by open-heart surgery for either valve replacement or repair in addition to excision of atrial myxoma versus 224 cases (27o/o) of closed cardiac procedures as closed mitral commissurotomy and pericardia! operations.
