Laryngeal Air Column Width Ratio in Predicting Post Extubation Stridor

Abstract


ost extubation stridor is one of the serious complication that can occur after liberation from mechanical ventilation and may have poor outcome.
Our study was conducted at RICU at Abbasia chest hospital from June 2018 to December 2018.Atotal 50 patients who mechanically ventilated more than 24 h and considered ready for weaning (hemodynamically stable and pass spontaneous breathing trial on PS mode of ventilator) were included.
CLT and laryngeal ultrasound to measure ACW after cough deflation were done immediately after intubation and 3 hours before extubation. Air column width ratio (before extubation\ after intubation) was calculated. All patients were observed for stridor occurrence and need of reintubation.
The aim of the work was to calculate air column width (ACW) ratio by ultrasonography and evaluate its diagnostic value in prediction of post extubation stridor (PES) in patients who mechanically ventilated more than 24 h.
The present study revealed the following results:
68% were males and 32%were females. The mean age was 58 ± 14.71 years SD. The main diagnosis was COPD, AE and RF type II (52%).
