“Prosthesis and Implant Survival in Immediately Loaded Full Arch Restorations Using Fiber-Reinforced Versus Non-Reinforced Temporary Frameworks: A Randomized Clinical Trial”

Abstract


The aim of this study was to compare between fiber reinforced temporary frameworks versus non-reinforced temporary frameworks in case of immediately loaded full arch restorations in completely edentulous patients regarding prosthesis and implant survival.
Completely edentulous patients were selected for the study and randomly allocated into two groups: non-reinforced group (control group) and fiber reinforced group (intervention group). Complete dentures were fabricated for all patients to be used as radiographic stent and conversion prosthesis for immediate load. Implant planning was done according to the “All-on-4 concept” and surgical guides were 3D printed. Guided surgical placement of the implants was done and the multiunit abutments with pick up titanium cylinders were used. Fiber reinforced framework was sealed to the Ti cylinders using flowable composite in the FR group and pick up of the conversion prosthesis was done by injecting self-cure acrylic resin in the opened window of the denture. On the other hand, pick up in the NR group was done by injecting the self-cure acrylic resin directly in the holes of the dentures opposite to the cylinders.
Digital periapical x-rays using paralleling technique were taken to the implants directly after implant placement. Patients were recalled after 4 months to evaluate prosthesis survival, implant survival and marginal bone loss. Data was collected and results were analyzed.
The results showed that fiber reinforced framework restorations have greater prosthesis and implant survival rate than the non-reinforced restorations. Results also revealed that the use of fiber reinforced frameworks significantly decreased the amount of marginal bone loss around the implants after 4 months compared with the non-reinforced group.
