Impact of Lipophilic Versus Hydrophilic Statin Administration on the Clinical Outcome and Cardiac Markers of Patients with Heart Failure

Abstract
There is evidence suggesting that lipophilic statins may be superior to hydrophilic statins in patients with heart failure. However, there are insufficient head-to-head comparison trials to confirm these arguments. The present study was aimed at comparing the effects of lipophilic statins against that of hydrophilic statins on heart failure patients. With regards to the following:
• Cardiac function& ventricular remodeling by estimating left ventricular ejection fraction and left ventricular volumes
• Cardiac & remodeling biomarkers
• Inflammatory biomarkers
• Quality of life
• Packer composite outcome
• Incidence of hospitalization
• Incidence of major adverse cardiac events
Therefore, to achieve this aim, this prospective randomized comparative study was designed. Patients presenting to the cardiac rehabilitation unit in Ain Shams University Hospital with heart failure, NYHA class (II-III) and ejection fraction ≤40 were randomly assigned to the following groups:
Atorvastatin group (n=42): These patients received their standard therapy for HF (based on guideline recommendations) and atorvastatin 40 mg once daily.
Rosuvastatin group (n=43): These patients received their standard therapy for HF (based on guideline recommendations) and rosuvastatin 20mg once daily.
