Abstract


Being edentulous is a challenging situation which affects the quality of life and has a real psychological impact, it is like being handicapped. 5
Nowadays prosthodontic treatment has been greatly changed by the use of implants for the edentulous patient. A variety of prosthetic designs associated with implant prostheses can be observed; new designs have emerged in response to the specific clinical conditions of the edentulous jaw provided with implants. Implant technology has valid clinical methods and treatment strategies that exist to ensure the quality of prosthetic reconstruction. Hence, with regard to some special implant indications, specific prosthodontic principles are needed.5
Loss of posterior teeth, at an early age particularly, leads to the loss of alveolar bone with a relative surfacing of the inferior alveolar nerve in the mandible, thus often prohibiting placement of implants in the posterior regions. Tilted implants could be used as an alternative, allowing maximum use of the existing bone and placement of posterior fixed teeth with minimum cantilevers, in a region where bone height and nerve proximity does not allow for the placement of axial implants.120
Moreover, using finite element analysis, it is conclude that using splinted tilted distal implants have a biomechanical advantage over axial implants when used to support distal cantilever units when comparing the coronal stress.120
Equivocally, implant-supported over-dentures (IODs) are an accepted and predictable form of treatment for the edentulous jaw. It have been revealed by clinical studies high survival rates for observational periods of up to 10 years and a high level of patient satisfaction, as well as an improvement in quality of life compared with conventional dentures. A wide variety of different IOD designs with various numbers of implants and different attachment types for the connection of implants and removable denture have been described. Abutment types included round or milled bars, ball attachments, magnet attachments, and double crowns have been commonly used. Double crowns has been suggested to provide better accessibility for carrying out peri-implant hygiene measures compared with bar constructions.38
