Evaluation of laparoscopic- assisted appendectomy Versus open appendectomy

Abstract


This present study included evaluation of the results of the laparoscopic assisted appendectomy versus open appendectomy regarding operative time, intraoperative complications, postoperative complications, hospital stay, post operative analgesia, as well as return of the patients to their normal activity.
This study included 40 patients (14 males and 26 females),who presented to Surgical Emergency Unit of Mini a university hospital in the period from March to July, 2006 with acute abdominal pain and diagnosed as acute appendicitis.
The patients (mean age 18-43years) were classified into two groups: First group (20 patients) : Open appendectomy group.
Second group (20 patients): Laparoscopic assisted appendectomy. Open appendectomy group.
In the Laparoscopic assisted group, a 10-mm port is inserted near the McBurney point, then complete exploration of the abdomen is done as well as the genital tract in females(uterus, ovaries and fallopian tubes). The appendix is grasped with a nontraumatic grasper, from the distal mesentery in line with the grasper, and pulled into the port for some
• distance. At the same time, the pneumoperitonium is evacuated to allow

approximation of the caecal. wall to the anterior abdominal wall. The grasper holding the appendix and the port are delivered into the port site. The rnesentery is grasped with a Babcock forceps and the port and nontraumatic grasper removed. Appendicectomy is carried out in the conventional fashion, by gradually delivering the whole of the appendix, and conversion from LAA to OA was carried in one patients (5%) due to
perforated appendix.
