“Effect of Monolayer versus Multilayer Leucocyte-Platelet Rich Fibrin (L-PRF) in Interdental Papillary Reconstruction” (A Randomized Controlled Clinical Study)

Abstract
Many factors have been identified for interdental papilla loss, leading to appearance of black triangles that are unaesthetic. In today's aesthetic driven focus in dental practice where both the dentist as well as the patient is becoming increasingly conscious about aesthetic needs, the real challenge is to construct this lost papilla and moreover to obtain stable and long term results.
L-PRF is frequently used in oral and maxillofacial surgery as a surgical adjuvant to improve healing and promote tissue regeneration. The L-PRF clot or membrane contains most of the platelets and half of the leukocytes present in the initial blood harvest.
A randomized clinical study was performed to compare the effect of monolayer and multilayer of L-PRF on interdental papillary reconstruction. Twenty systemically healthy patients with Nordland and Tarnow class I and II papillary recession participated and completed the study. Parameters as distance from tip of the papilla to contact point, plaque index, gingival index and volumetric assessment were assessed at baseline. A single vertical incision was done, undermined, coronally displaced and then L-PRF was placed in the incision and sling suture was done. Patients were regularly followed up and evaluated with the parameters recorded at 2 weeks, 3 months and 6 months. There was no reduction in the black triangle area in both treatment modalities and no change in volume between baseline and 6 months. All patients felt no cosmetic change and won't repeat the procedure if needed.
