Abstract


Graft-versus-host disease (GVHD) continues to compromise the overall success of allogeneic hematopoietic cell transplantation (HCT). Although rates and severity of acute GVHD have decreased with improvements in donor selection criteria, pharmacologic prophylaxes, and supportive care, rates of chronic GVHD have remained remarkably stable at 35% to 50% for many years.
Interleukin 6 is a cytokine associated with several inflammatory diseases and a modulator of the immune responses involved in acute Graft versus Host Disease pathogenesis. It is elevated in the circulation after allogeneic hematopoietic cell transplantation (HCT). Elevation in interleukin 6 plasma concentration is associated with the onset of and poor survival after acute GvHD.
IL-6 plays a role during early GVHD development. Blockade of the pro inflammatory effects of IL-6 seems to be a possible therapeutic strategy since IL-6 and STAT3 activation are closely linked to the development of both Th17 and T reg cells, and since early STAT3 phosphorylation post-transplant seems to precede development of GVHD.
