Abstract


The aim of the present work was to compare splenectomy with devascularization and sclerotherapy in the treatment of patients . with chronic liver disease and esophageal varices with a history of upper gastrointestinal bleeding.

One hundred patients divided randomly into two groups, splenectomy with devascularization was done to 50 patients while the other 50 patients were subjected to sclerotherapy.

Patients of the two groups were subjected to careful detailed history taking, clinical examination, laboratory investigations, viral markers for hepatitis B & C viruses, upper gastrointestina I endoscopy . and abdominal ultrasonography.

Child score was calculated, and the aetiology of the underlying liver disease was determined either by needle liver biopsy or ultrasonography. The two groups were matched as regards clinical examination, laboratory investigations and the grade of esophageal varices.

The patients of the surgical group were evaluated at one month, three months, six months, one year after the operations and every year after, the patients were subjected to the most of the preoperative work up in the last follow up.
