EVALUATION OF SOME DIAGNOSITC AND THERAPEUTIC ASPECTS OF PORTAL HYPERTENSION IN CHILDREN

Abstract
Portal hypertension (PH) in children is a serious problem and children with such condition need periodic evaluation of the portal venous system (PVS) hemodynamic changes which can be assessed by the recent non-invasive duplex Doppler ultrasonography (US).
Upper gastrointestinal endoscopy is the best method of visualizing esophago-gastric varices as well as gastric mucosal lesions which are common in patients with PH and constitute important sources of gastrointestinal bleeding in these patients.
Reduction of PH could be achieved by continuous medical treatment using propranolol that effectively lower portal pressure and improve the portal portal hypertensive gastropathy (PHG) in patients with PH.
The aim of this work was to study the hemodynamics of the portal and splenic veins (PV and SV) using the duplex Doppler US in children with PH to find the relation between these parameters, intrasplenic pressure and the occurrence of esophageal varices and portal hypertensive gastropathy assessed both endoscopically and histopathologically as well as to evaluate the effect of propranolol therapy on these parameters.
This study was conducted on 25 randomly selected children (12 males and 13 females) suffering from portal hypertension of different etiologies as suggested clinically and documented by measuring portal pressure that exceeded 20 em saline using transplenic manometer. Their ages ranged from 3 to 15 years. They were selected from those admitted to the Pediatric Department of Tanta University Hospital. Ten normal children of matched age and sex were studied as a control group for clinical, laboratory and ultrasonographic parameters.
