Role of Human Leukocyte Antigen in Liver Transplantation and Its Relation to Outcomes Thesis

Abstract

SUMMARY
L
iver transplantation is the treatment of choice for patients with decompensated cirrhosis, acute liver failure, Hepatocellular Carcinomas (HCCs) (Lucey et al., 2013).
The exact role of HLA compatibility between donor and recipient in organ transplant outcomes has been widely studied. The advantageous effect of HLA compatibility has been recognized in kidney (Takemoto et al., 2004) and heart (Kaczmarek et al., 2006) however, the importance of HLA compatibility in the long-term results of liver transplantation has not yet been clearly defined. Thus, prospective HLA histocompatibility analysis is not routinely performed before liver transplantation, and has not been used for the allocation of liver grafts (Shin et al., 2016).
Early observations revealed that liver allografts were resistant to hyperacute antibody- mediated rejection, and this led to the question whether a cross match result was relevant to or required for hepatic transplantation (Iwatsuki et al., 1981).
The aim of this work was to assess HLA compatibility, HLA antibodies and cross matching in liver transplantation recipients and their relation to acute rejection, CMV infection, and recurrence of HCV.
In the present study conducted on 24 patient
