Role of MRI Guided Biopsy of Suspicious Breast Lesions

Chris Nabil Hanna;

Abstract

Breast Cancer prevalence has been rapidly increasing over the past few years.Becoming the second most common cancer among women. Thus necessitating the tremendous advances in diagnosis and treatment.

The superiority of MRI over the other conventional methods in breast imaging has been repeatedly proven. Due to its high sensitivity and specificity many lesions could only be detected by MRI.
Even 2nd look ultrasonography or sono-mammography came short in detecting those lesions, making them candidates for MRI guided breast biopsy.

Mainly in patients that are difficult to evaluate as those with breastimplants, young females with dense breasts, especially those in high risk groups or with a positive family history.
Also MRI has the advantage of detecting lesions in the contralateral breast and the missed lesions in the ipsilateral one.

The primary objective of breast MRI is the detection of clinically and radiologically occult lesions at an early stage. To plan an organized approach for patient treatment before it has metastasized to other organs. Accordingly improving morbidity and mortality and the quality of patient’s life.
Tools such as MRI Spectroscopy, contrast intensity curves and high fields may in the future increase the specificity of MRI allowing better selections of lesions to be biopsied and the detection of possible post-biopsy residual tumour.
MRI guided breast biopsy could be approached by many devices vacuum assisted, fine needle and automated core biopsy devices. MRI-guided vacuum-assisted breast biopsy is fast, safe, and accurate. It is the most largely and widely used. Although it does pose some challenges, yet most can be overcome.
Meticulous preparation, careful positioning, and directional sampling are the key to a successful biopsy. A dedicated experienced team allows for seamless progression of the biopsy.Being able to successfully perform MRI-guided interventions is integral to a breast imaging program
