Doppler Values of Cerebro Placental Perfusion and Amniotic Fluid Index In Postterm Pregnancy

Abstract


Prolonged (postterm) pregnancies are associated with both fetal and maternal complications. A variety of management practices can be used to overcome the risk of these complications.
Technological advances in Doppler velocimetry allowed the study of other fetal vessels such as middle cerebral artery (MCA) improving detection of disturbances in fetus wellbeing.
The aim of this prospective study was to evaluate the value of Doppler velocimetry of middle cerebral and umbilical arteries and amniotic fluid volume in the surveillance of postterm pregnancies.
This study was carried out on two groups each one included 54 patients , one group from post date (41-42 weeks) and other group from (37-40 weeks) attending the antenatal clinic at Ain Shams University maternity hospital.
Our results showed no significant differences as regard maternal age, body weight; gravidity, and parity between postdate group and control group and no statistically significant difference between groups according to Doppler indices [Middle cerebral artery pulsatility index (MCA-PI); Middle cerebral artery resistance index (MCA-RI); Umbilical artery pulsatility index (UA-PI); Umbilical artery resistance index (UA- RI)] & regarding the mean values of Cerebro placental ratio (CPR).
As regard amniotic fluid volume (AFV), our study showed a highly statistically significant difference between postdate and control group.
Analysis of the delivery data in the two studied groups in our study as regard mode of delivery showed that (55.6%) of cases in postdate group and (78%) of cases in control group were delivered by vaginal delivery, while (44.4%) in postdate group and (22 %) in control group were delivered by caesarian where (62.5%) in postdate group and (8.3 %) in control group were delivered by caesarian due to fetal distress, while (20.8%) and (41.7%) were delivered by caesarian due to failed induction respectively and (16.7 %) and (50 %) were delivered by caesarian due to failure of progress respectively. There was significant difference between postdate and control groups as regard mode of delivery.
