THE RELATION BETWEEN BODY MASS INDEX AND LEFT VENTRICULAR MASS AND CARDIAC OUTPUT IN PATIENTS WITH CHRONIC HYPERTENSION

Abstract

The correlates of cardiovascular abnormalities in relation to the level of obesity in hypertensive obese patients remain unclear. So, this study was conducted aiming at investigating the relations of BMI to LVM and COP in patients with essential hypertension. Also, the study aimed at estimating the prevalence of LVH in the studied subjects. The study was conducted in the department of internal medicine, Sohag University Hospital. A total of 120 subjects were recruited during the study period. They were divided into three main groups : group I (control group) included 30 normotensive normal weight subjects; group II incluqed 30 hypertensive normal weight patients; and group Ill included 60 hypertensive obese patients. The latter group was further classified into two subgroups according to WHR: group lila with gynecoid obesity (20 patients) & group lllb with android obesity (40 patients). in addition to the anthropometric measurements , all subjects had lipogram, ECG & examined by echocardiography . The three groups were age matched ( the age ranged from 40 to 60 years), sex matched ( female to male ratio was about 2:1), and showed no significant difference in their residence. The hypertensive patients had comparable duration of hypertension & heart rate, but the mean BP was. increased in group II patients. The heart rate was significantly increased in hypertensive patients compared to normotensive subjects. Significant increase in LDL-c & TGs, and significant decrease in HOL-e was evident in group Ill patients compare to Group II, however both groups had comparable total cholesterol levels. Hypertensive patients with android obesity patients showed significant increase in LDL-c level than gynecoid obesity patients. The other lipogram parameters were comparable.
