Medication Adherence and Treatment Satisfaction in Chronic Obstructive Pulmonary Disease and Bronchial Asthma

Abstract


Medication adherence is one of the important factors that can determine the therapeutic outcome.26 Compared with other chronic conditions, asthma and COPD treatment have markedly lower rates of adherence.28 Higher satisfaction was generally associated with greater adherence, but the strength of this relationship varied.39 Ensuring treatments are satisfactory to the patient may promote greater adherence, but the strength of the impact is likely to vary across populations.39
The purpose of the present study was to assess the medication adherence and treatment satisfaction among some Egyptian patients with COPD and bronchial asthma.
This study was conducted upon 496 patients at Qena Chest Hospital from 1/1/2014 till1/1/ 2015; 225 patients were asthmatics while the remaining 271 were having COPD.
The validated Arabic version of Morisky medication adherence scale(MMAS) and Treatment Satisfaction Questionnaire for Medication (TSQM 1.4) were used in this study obtaining the license agreement for obtained for these questionnaires.
The following results were obtained:
1-COPD patients were less adherent to their medication than asthmatics.
2- All TSQM subscales (including effectiveness, side effects, convenience, global satisfaction) were significantly lower among COPD patients in comparison to asthmatics.
3- All TSQM subscales were significantly higher among male in comparison to female in both COPD and asthma group .
4-Medication adherent were significantly lower among the current cigarette smoker COPD patients whereas side effects and convenience were significantly higher among the current cigarette smoker COPD patients.
5-COPD patient with recurrent hospital admission were significantly more adherent to their medication and less effectiveness and side effects in comparison to those without recurrent hospital admission.
6- All TSQM subscales were significantly lower among asthmatics with recurrent hospital admission in comparison to those without recurrent hospital admission.
7- COPD patient with comorbid medical condition were significantly more adherent to their medication and less effectiveness, global satisfaction and convenience in comparison to those without comorbid medical condition.
8-Co morbidity among asthmatics didn’t affect the medication adherence yet, these asthmatics without co morbidity showed significantly higher effectiveness, global satisfaction and side effects to their medication.
9- For asthmatic patients, there were significant positive correlations between the subscales of TSQM and each other, MMAS scale & pulmonary functions. There were significant negative correlations between total MMAS scale and duration of general& current chest
