THE USE OF LOW DOSE TOTAL BODY•IRRADIATION IN TREATMENT OF RELAPSED NON HODGKIN•s LYMPHOMA

Abstract
At NCI, Cairo, lymphoma constituted 6% of all cancers. In patients refractory to or relapsing qfter initial therapy, the 2"d line chemotherapy may give response however, it is short-lived and long-term survival is rarely seen. LTBI seem to provide an alternative mechanism of action against NHL cancer cells.
AIM OF WORK
The principle objective of the trial is to assess the therapeutic activity of LTBI in patients with relapsed NHL with assesment of the effects of LTBI on various subtypes of lymphocytes (in blood and tissue cytology). The 2nd objective is to characterize the acute side effects of LTBI.
MATERIAL AND METHODS
Seventy seven patients with relapsed or resistant NHL after chemotherapy included in the study.The patients will receive LTBI in the form of 2 cycles separated by 2 weeks rest. Each cycle formed of 80cGyl 4 fractions/ over 4 consequetive days. IF radiotherapy will be given to initially bulky sites or residual disease after 2 months from the end of LTBI. Multiple blood samples and cytology will be withdrawn before and after each treatment cycle and tested for the various subsets of lymphocytes and apoptosis.
RESULTS
The response documented in (71.4%) with median time to progression of the disease 10 months. The median over all survival duration was 10 months calculated from date of LTBJ. Seventeen patients only were still alive. The 4 year OAS was 20±5%. 22177 entered CR The median disease free survival was 17 ± 7. As expected, the LTBJ toxicities encountered were mainly haematological (58% of cases). The percentage of CD5, 4, 8, /9, 95, /6, 56 also the CD4/CD8 ratio increased by adding LTBI and the mean value decreased as a consequence of decrease total leucocytic count by adding LTBJ. The mean value of apoptosis measured by annixine before LTB/ was 4.04 ± 5.6 compared with 5.54 ± 7.1 after LTB!.
CONCLUSION
LTBI gives comparable results to chemotherapy in relapsed NHL. LTBI is cheap and tolerable. LTBJ is as effective as it is in high grade NHL compared by low grade NHL..
