Comparison of regenerated peri-implant bone defect around endosseous immediate dental implant using (DFDBA) versus DFDBA and platelet gel

Abstract


The atm of the present study was directed to evaluate clinically, histologically and by mdiographic analysis the effectiveness of platelet rich plasma (PRP) plus DFDBA versus DFDBA alone in treating peri-implant bone defects around immediate endosseous dental implant. This study design included clinical and experimental parts. The implants were installed in the extracted sites of hopeless teeth in any areas from lower right bicuspid to left bicuspid. The patients were randomly classified into 2 groups. The control group received immediate implant and intrabony defects were augmented with DFDBA. In the tested group the bony defects were augmented by DFDBA and PRP. Periodontal parameters were caiTied out at 4 and 9 months post implantation and radiographic evaluation were caiTied out to estimate the outcome of regenerative materials. In the experimental design, 12 implants were placed in 6 dogs bilaterally. In control sites 6 fresh sockets with surgical induced defects replaced by immediate implants and augmented with DFDBA. The other 6 tested sockets were replaced by immediate implants with DFDBA and PRP. Two dogs were sacrificed at I, 3 and 6 months post operative and were evaluated histologically.
The results demonstrated that the tested sites which treated with DFDBA with

PRP showed more significant improvement in terms of the clinical parameters compared to control sites at different follow up periods. Assessment of implant shoulder alveolar bone distance (DIB) showed a statistically significant increased in the mean and the percentage of bone fill at 4/9 months postoperative in the tested group. In addition, there was a significant increase in the mean of bone subtraction value in the group treated with DFDBA and P RP at the baseline/4 months and at 9 month (P< 0.01) with no statistically significant difference at the 4/9 months period (P>0.05). The histological and histomorphometreic findings revealed a more thickness of new bone formation at implant bone interface in the tested group than that of the control at all the periods of evaluation. The difference was clearly observed one and three months post implantation. This study provided experimental and clinical evidence in support of use of DFDBA and PRP with immediate placed implant particularly in presence of bone defects.
