Abstract


Morbid obesity is one of the medical problems needed much care the deal with it. Many systemic diseases are present in association with obesity. The cardiovascular system and the respiratory system are the most important and most commonly affected systems.
Morbid obesity is a major risk factor for developing high blood pressure, high blood cholesterol and atherosclerosis, all of which are risk factors for cardiovascular disease including heart failure and coronary heart disease.
Morbid obesity is associated with respiratory disorders during sleep including sleep apnoea. An association between overweight and asthma has also been established.
Other systems that might affected by obesity including the renal system , the endocrine system .the digestive system and the immune system. Also there is a real relationship between cancer and obesity.
As the prevalence of obesity continues to rise in society, an increasing number of the patients undergoing surgery will be obese. Obese patient may undergo bariatric or non bariatric surgery Laparoscopic Roux-en-Y gastric bypass and laparoscopic adjustable gastric banding are the most commonly performed weight-reduction operations in the world. Patients undergoing these operations may have many complications which may be general as bleeding, thrombo-embolism and infection or specific as gastric leak, perforation and peritonitis. Also obese patients undergoing non bariatric surgery have post operative "pulmonary complications" as pneumonia and pulmonary embolism and "cardiovascular complications as arrythmia especially atrial fibrellation .
Great care should be taken in the preoperative assessment. Careful history, clinical examination and investigations should be played more towards cardiovascular disease, pulmonary function and pulmonary diseases, as well as endocrinal abnormality including thyroid functions and diabetes mellitus.
Caring for critically ill severely obese patients is a challenge for the entire ICU staff. Management of the post operative complications may require specific hospital care needs, the appropriate thrombo-prophylaxis and the proper diagnostic procedures. Also may require estimating dose for analgesia, proper nutrition, airway management and skin care.
