Role of MRI in Evaluation of Traumatic Ankle Injuries

Abstract


The ankle is the most frequently injured joint of all major weight bearing joints in the body. Ankle injuries account for 25% of athletic injuries.

Ankle pain associating trauma is a common complaint in orthopedic practice. The etiology of ankle pain includes various pathologies such as tendon and ligament injuries, joint disorders, osseous lesions, and soft tissue pathologies.

Many ankle injuries were undiagnosed by conventional radiology and needs further MRI evaluation to diagnose ligamentous, tendinous and muscle injuries as well as osseous lesions as stress fractures, osteochondral lesions.

This study included 40 patients (28 Males, 12

Females) presented with post traumatic ankle pain. MRI examination was performed in all cases, and the result found 70 ankle injuries were classified as following:

- Joint Effusion: Which is the most common pathology found on MRI representing (32.8%) of all encountered pathologies.

- Ligamentous Injuries: Which form (24.3%) of all encountered pathologies, and The ATFL was the most frequently torn ligament.

- Tendon Injuries: Which form (22.9%), Flexor Halluces Longus tendon tenosynovitis and Achilles tendon injuries were the most common tendon injuries found, denoting that the medial and posterior tendon groups injuries are most common cause of the pain following ankle joint trauma and it is diagnosed easily by MRI.

- Bone Lesions: Which form (20%) of all encountered pathologies, and belong to Bone Lesions, the Osteo- Condral lesions form (35.71%), fractures were (21.43%) and Bone Contusion (42.86%).

So, we can conclude that MRI is modality of choice in evaluating ankle injuries due to its high soft tissue contrast resolution, and multi-planar capabilities. It provides a non-invasive tool for the diagnosis of Ankle injuries, which are often difficult to diagnose with alternative modalities. MRI is particularly advantageous for assessing soft tissue structures around the ankle such as tendons, ligaments, nerves, and fascia and for detecting occult bone injuries.
