DYSRHYTHMIA IN POSTOPERATIVE PATIENTS

Abstract


Dysrhythmia is broadly defmed as any normal or abnormal heart rhythm at inappropriate rate for circumstances or with ineffective atrial or ventricular contractions without proper AV conduction. Although arrhythmia is a misnomer, it is used more frequently than dysrhythmia.


The specialized tissues of heart that initiate the cardiac impulse and conduct it consist of sinus node, internodal tracts, atrioventricular junctional area, bundle branches and terminal Purkinje fibers.


The sinus node possesses spontaneous self-initiated electronic activity taking the upper hand as pacemaker of the heart, while other parts of the conductive system also have this property but become suppressed by overdrive suppression and being latent pacemakers.


The mechanism• of initiation of arrhythmia depends on one or both mechanisms, first, abnormal impulse initiation, second, abnormal impulse conduction.


In postoperative period usually the cause is related to non­ cardiac origin. However, presence of organic heart disease such as mitral valve disease, ischemic heart, cardiomyopathy, etc... acts as substrate for arrhythmia. Many of these non-cardiological factors are identified including postoperative pain, hypovolemia, hypervolemia, fever, hypothermia, anemia, hypoxia, hypercarbia, pulmonary emboli, anxiety, hypertension, hypotension, bladder distention and electrolyte disturbances especially hypokalemia
