Brain Support and Brain Tissue Oxygen-Directed Management and Out Come after Head Trauma
Bahaa Maher HatemAttia;
Abstract
Head trauma isa major catastrophic event that leads to severe disability up to loss of life of persons. And the damage of the head trauma can be classified into primary and secondary stages.
Favorable outcome can be supported by early and rapid protection of airway and breathing with well oxygenated airway and maintaining well circulation by means of intravenous fluid. Assessment of head traumatized patient done by GCS and the less the score the poor the prognosis, with CT scan types of lesion can be determined.
Meticulous and continuous monitoring of vital data and ICP is a key in preserving well balanced CPP in order to preserve healthy brain tissues and decrease the damage resulted from head trauma.
The management is decided and designed according to each lesion need either therapeutic by positioning, hyper ventilation and hyperosmolar or surgical for skull fractures and evacuation of hematoma.
Brain tissue oxygenation in general and specific is the major determinant of prognosis. Well applied methods to improve (PbtO2)by decreasing incidence of hypoxia and maintaining well supported circulation lead to favorable outcome after head trauma.
