CLINICAL AND RADIOLOGICAL OUTCOME OF ENDOVASCULAR COILING OF ANTERIOR CEREBRAL CIRCULATION ANEURYSM

Abstract
Background and Purpose: A precise analysis of the immediate postoperative anatomic results and clinical outcome in a large series of anterior cerebral aneurysms treated by endovascular approach has not previously been presented. This study aimed to assess the efficacy of endovascular treatment of anterior cerebral aneurysms in light of immediate postoperative anatomic results in a prospective in Ain Shams hospitals.
Methods: We performed a prospective study of cerebral aneurysm cases that were referred to our hospital and underwent endovascular treatment. After endovascular coiling, the patients were followed up at 6th , along with a secondary angiographic study in 6th month . The outcome and adverse event of the treatment were studied.
Results: Out of 40 patients studied between 16-65years at age, about 87.5% had ruptured aneurysm, The mortality rate was 0%. The rate of radiological outcome after procedure was Total or subtotal occlusion of the aneurysm was performed in 90% of patients and complications of procedure were of 10%cases with perforation and bleeding and about 12.5% of cases with thromboembolic manifestation.
The procedural results remained stable in 94% of patients in the secondary angiographic study at 6th month follow up and there were about 97% of GOS 5 grade.
Conclusion: We demonstrated the feasibility and clinical efficacy of endovascular coiling in management of cerebral aneurysms. The outcome of the endovascular coiling is better in anterior cerebral aneurysms and is worse in patients with worse clinical presentation and those with failure of endovascular procedure in complete occlusion.
